1. Is this legal?
Yes.Horizon Medical Care provides you an online telemedicine portal in which you can be assured you are receiving prompt, courteous, professional - and most of all, LEGAL - telemedical consultations. Adjunctive medical consultations via telemedicine are legal when performed within the established federal, state, and local regulations including U.S.P.L. 104-191 § 1171.2, HIPAA, DEA 191-N, and in accordance with such governing bodies as the FDA, DEA, U.S. Customs Service, U.S. Department of Health & Human Services and the American Medical Association. We at HMC have taken painstaking measures to ensure compliance with all federal and state laws to provide our members telemedical consultations.     back to top

2. How much does this service cost?
The cost of a telemedical consultation is $200.00 for script mailed directly to you.  However, you will not be billed for any consultation until after you have received it, and only if your consulting physician determines that prescription treatment is necessary to relieve your complaint. If you are denied treatment following a consultation, you will be billed a $25.00 administrative fee for the costs associated with preparation and storage of your medical records.  We offer 20% discount to VIP members.     back to top

3. Are there any "hidden charges"?
No. The only costs which will be billed to you by HMC are:
  • $200.00 for the consultation
  • Applicable fees for rescheduling or canceling a consultation within hours of the date of your appointment (Please see our cancellation policy for specific fees.)
  • $25.00 administrative fee – You will only be charged this fee if you are denied treatment following a consultation.

4. How does telemedicine work?
Telemedicine is the integration of information technology into health care services.

The HMC Telemedicine Process is comprised of five steps:
  • STEP 1 - REGISTER : You must simply submit your name and a current email address to begin the process. This email address will become the "foundation" of your HMC account.
  • STEP 2 - BECOME A MEMBER : Once you have registered, an email with detailed instructions for gathering and submitting your medical records.
  • STEP 3 - REQUEST A CONSULTATION : you select the best date and time for your appointment with our consult scheduling tool.
  • STEP 4 - YOUR CONSULTATION : an HMC health care provider will call you at the time you chose on the date of your appointment. You will privately consult with the doctor who will then determine the most effective treatment modalities for you.
  • STEP 5 - EVALUATION : Afte the doctor performs the consultation with you and if he/she determines that you are not a candidate for medication therapies he/she will advise you as to the best follow up plan and treatment options. HMC is a referral service that puts the patient in touch with a qualified consulting physician and can not guarantee that medication will be the appropriate treatment plan.
  • STEP 6 - PRESCRIPTION : if prescription medications are written as part of your treatment, (1) the prescription will be automatically sent to an HMC pharmacy affiliate to be filled and shipped; (2) have the script directly mailed to you. 
After your membership is activated, you will be able to choose your own consultation dates and times, all from the comfort and privacy of your own home in a matter of minutes.     back to top

5. What do I get in return for the costs?
Hopefully, you ultimately experience relief from the medical condition(s) affecting you. After your profile has been processed and formatted for telemedicine, HMC provides you an easy to use, secure digital system for storing your medical records. As a member of HMC, You will always have access to our array of unique and powerful web-based tools which allow you to manage your account information and to consult with a physician anytime you choose, from the comforts of your own home, or wherever else you may be; even while you are traveling. 6. What are the risks of using telemedicine?
The risks of telemedicine only come from using services that do not operate within the law. HMC requires valid state issued identification and copies of medical records to authenticate each client's identification and to ensure that proper treatment, if any, is prescribed. We also protect all confidential client information in accordance with HIPAA.

Some services do not require medical records or identification of patients before dispensing prescriptions; only requiring you to complete an online questionnaire. According to the standards of care outlined by the American Medical Association and enforced by the DEA and other law enforcement agencies, prescribing and dispensing medication in this way is not neither ethical nor legal.

It is possible to order prescription drugs from companies outside of the United States; however this is both illegal and potentially unsafe. Importation of prescription medication is simply not legal without the proper licensure. It is often difficult to resolve disputes across international boundaries concerning payment and receipt of foreign goods, especially when those goods are offered in direct violation of federal laws. Please be very careful in deciding who you entrust your personal health care to, even if it is not HMC.     back to top

7. How does HMC use my personal information and medical records?
HMC uses your personal shipping and billing information to establish your account and to provide you the goods and services which you request. We also format and deliver your medical records electronically through our secured network to those covered entities directly involved in your health care.

HMC protects ALL of your personal and medical information with 128-bit Secure Socket Layer (SSL) data encryption and does not resell or authorize access to your information for any outside party who is not a covered entity directly involved in the cycle of your health care, except where required by law. All email communication between HMC and its clients is conducted individually and securely. We do not send mass-email communications, nor do we permit such mailings to our clients by third-parties. 8. Why should I use HMC instead of local doctor?
There are many reasons you may wish to use HMC, including:

INCREASED PRIVACY - Not only is your information secure, but you also benefit from being able to speak with your doctor without having to arrive early at his office only to sit and wait. This increased privacy and added convenience allows you to continue living your life while being able to seek medical advice and treatment.

INCREASED APPOINTMENT AVAILABILITY - Availability of physicians is of utmost concern to HMC. Nothing is more frustrating than needing to talk with your doctor and having to wait weeks for the first available appointment. When you schedule a consultation with HMC, you can immediately see all available appointment times and you can choose the one that works best for you. We strive to provide our clients the greatest availability of physician consultations possible to allow you to speak to a physician whenever and wherever you choose.

LOCATION - since you no longer have to travel to your doctor, you can consult with your HMC physician from home, from your office, or even while you are on vacation or traveling.     back to top

9. Payment methods
HMC accepts all major credit cards including Visa, MasterCard, American Express and Discover, as well as COD, Cashiers Check, and Money Orders.  We do not accept personal checks. We apologize for any inconvenience this may cause you.     back to top

10. Cancellation policy
We understand that “life happens” and that you may need to reschedule or cancel an appointment. Once you request a consultation, you may reschedule or cancel that consult anytime up until the hour prior to your appointment. If you must reschedule or cancel an appointment, you will not be charged unless it is within 48 hours of the date of your consultation, as follows:
  • Within 48 hours of the day of consultation = $15.00 administrative fee
  • Within 24 hours of the day of consultation = $30.00 administrative fee

11. What services does HMC provide?
HMC provides Board-certified physician consultations via telemedicine. In order to facilitate this service, we enable you to:
  • update and maintain your own account information
  • format and store your personal medical information on HMC secured servers
  • schedule consultations at your convenience
  • consult with physicians and other health care providers

12. What is the role of HMC in telemedicine?
HMC is a telemedicine service provider known as a "Health Care Clearinghouse". Part of our mission is to receive, process, and format your profile and medical records and to present them to telemedicine health care professionals for treatment consideration. We also maintain member consultation scheduling; physician and physician’s assistant availabilities; patient billing; and transmission of your treatment information to pharmacy affiliates and other health care providers for your treatment.     back to top

13. How do I receive my prescriptions?
HMC offers two options, if your treatment involves prescription medication, your script will be sent  directly to your home. Once you have provided us with a complete file, it should take one day or same day to receive a consult, one to two days for doctor review and script approval, one day for the script to be sent to HMC and then the script is sent to you next day. Please note, it has been known to take the doctor an extra day for approval if needed or a request for more documentation is necessary.     back to top

14. What is a pre-existing condition?
A pre-existing condition is defined as any medical condition:
  • which was diagnosed and documented no less than three months ago and no more than two years ago
  • for which you have previously received documented treatment while under the supervision of a physician
  • for which you have substantial medical documentation, not including medical bills
As it is impossible for HMC physicians to diagnose new medical conditions via telemedicine, you can only receive treatment for a condition which has been previously diagnosed by a physician.

If your condition was diagnosed within the past three months, you must have accompanying records to substantiate that the condition is chronic. All medical records must be written by a Doctor of Chiropractic (D.C.), Medical Doctor (M.D.), Doctor of Osteopathic Medicine (D.O.), or a Nurse-Practitioner under the direct supervision of a physician.     back to top

15. What is telemedicine?
According to the United States Health Resources and Services Administration, telemedicine (also referred to as telehealth) is defined as:

"...the use of electronic information and telecommunications technologies to support long-distance clinical health care, patient and professional health-related education, public health and health administration."     back to top

16. What measures does HMC take to ensure security and maintain my confidentiality?
HMC employs 128-bit SSL encryption to protect the transfer of all information through this web site. Additionally, we do not resell or grant access to any party not directly involved in your treatment without first notifying you in writing, except as outlined in our privacy policy. 17. What happens if I am denied treatment by an HMC physician?
It is more important to us that you receive sound medical advice than unnecessary prescriptions. If your medical complaint does not require treatment by prescription, you will not be billed for your consultation; however, you will be billed an administrative fee of $25.00 to cover the costs associated with formatting and processing your records for telemedicine.     back to top

18. Does HMC accept medical insurance?
Unfortunately, we do not currently accept medical insurance. As telemedicine is still an emerging industry, most insurance companies do not currently support telemedical consultation. You can, however, seek reimbursement from your insurance company on your own for services rendered to you by HMC.     back to top

19. Who are HMC' Physicians?
HMC contracts with board-certified physicians nationwide. In order to provide the best medical service to our members, we have contracted with some of the best physicians in the nation and in the telemedicine industry. We enable you to consult with any physician who is legally licensed to practice medicine in the state in which you live.     back to top

20. Can I receive consultations and/or prescriptions while I am away from home?
Yes. As you confirm the details of your consultation, you will be prompted to confirm your shipping and billing address. You will be able to update your billing and/or shipping address so that you are able to consult with an HMC health care provider and receive prescriptions even if you are away from home.     back to top

21. What states do you offer telemedicine consultation services in?
Currently, HMC has physicians who can provide telemedical consultation in every U.S. state except for Tennessee, Missouri, Nevada, North Carolina, West Virginia, Kentucky, Michigan and Mississippi.     back to top

22. What are the benefits of membership in HMC?
There are many benefits to membership in HMC. Chief among them is your ability to consult with board-certified physicians anywhere and at any time you choose. HMC ensures that your private information stays private - that information regarding your medical history and treatment is secure, and that it is not made available to anyone not expressly involved in your treatment, in accordance with HIPAA regulations.     back to top

23. Why can't I share an email address with another HMC member?
To ensure each client's privacy we require that each account uses a unique email address. This allows you to communicate with HMC regarding your private medical history and treatment without the possibility of another person gaining access to your information.     back to top

24. What if I don't have my own email address?
If you do not already have a personal email account, you can easily create a free account at any number of common web sites. You can sign up for s free email account through the Yahoo website at http://www.yahoo.com or http://www.msn.com .     back to top

25. I've forgotten my password ... how do I retrieve it?
Simply click on the "forgot your password?" link at the top right of each page and enter the email address on file for your account. Your password will be emailed to you.     back to top
