How It Works

The Horizon Medical Care process is…well…simple.

Step 1 - Register with Horizon Medical Care
If you have never registered with Horizon Medical Care, please go to the
homepage and then click on 'Register' to set up a new account. After completing the form, go back to the homepage. Horizon Medical Care will track each customer using their e-mail address and password.

*Note: If a copy of your identification, medical records, and documentation of your recent physical exam (within the last 180 days), are already on file at Horizon Medical Care you do not have to resubmit them. In order to complete your registration with Horizon Medical Care, you must provide copies of the following documents:

A. A copy of a state or government issued identification card, driver's license, military ID or passport.

B. Pertinent medical records, lab results, reports and findings for your current medical complaint and/or documentation you have been physically examined within the last 180 days signed and dated by your local physician. Your medical records must reflect the medical condition you are requesting a consultation for. You can fax the records to 1-205-278-8729.

*Note: We do not recommend treatment for anyone under the age of 21 and to patient residing in the Tennessee, Missouri, Nevada, North Carolina, West Virginia, Kentucky, Michigan and Mississippi.

Step 2 - Receiving Your Patient Registration Confirmation.
Once Horizon Medical Care has received copies of your identification and medical records, we will process your registration. If your registration is approved, you will receive an email which contains your unique HMC Patient ID number. (If you are not approved, you will receive an e-mail providing further instructions.)

*Patient ID numbers are confidential. Horizon Medical Cares Policy does not allow registered patients to share their patient ID number. Violation of this policy will result in the loss of all privileges with Horizon Medical Care.

This email address will become the foundation upon which your account is built. Once you login, proceed to the "Book A Consult" link and create your profile by filling in your patient medical history information, shipping and billing info. While we do collect your billing information in your membership profile, you will never be charged until after your consultation.

Step 3 - Setting Up the Consultation
HMC makes it easy for you to set an appointment with a board-certified, licensed US health care professional by using a dynamic scheduling tool that allow you to review the calendar of available appointment times and select the time which suits you best. You will also receive a confirmation e-mail outlining the consultation time and the telephone number for you to call the physician. All times shown on the calendar is Eastern Standard Time, and will be assigned at a time that has been scheduled per your request. Horizon Medical Care requests that you are prompt with your telephone appointments.

*Requirements:  To fulfill DEA requirements you must have MSN messenger and a web cam to consult with our physicians.  If you do not have MSN on your computer please click the MSN logo to download the free software.
